What should our church do if a plaque has a mistake or damage?

When a congregation is not happy with its order, the approved remedy is a 100% satisfaction refund. Say what went wrong and which Sunday the piece is meant for.

What should the church office send with a problem report?

The order reference, plus the congregation or ministry name used when the order was placed.
Which plaque is affected, named by the person honored, the ministry or the donor board row.
Photos of the plaque in daylight and, for transit damage, the carton and packing.
The line on the signed-off proof that differs from the engraving, if wording is the issue.
The date of the service, banquet or dedication where the piece is meant to be presented.

Damage or missing items: Keep the packaging, photograph the package and contents, check the packing list and share the delivery date.
Changes after approval: Report a changed “Year” line, date or dimension at once. Whether it can still be made depends on the stage of work and the order terms; any revised cost or schedule is confirmed before proceeding.
Remedies and refunds: Parish Recognition explains the remedy, any return instructions and processing under your order terms and statutory rights. Wait for those instructions before sending anything back.
